DXCC application form

..И вот наконец настал тот радостный момент когда вы готовы заявить один из  почетного семейства  дипломов  DXCC. Все мытарства с QSO и  с QSL позади и остается всего лишь составить заявку.
       И тут возникают неприятные нюансы. На сайте ARRL (которая и выдает этот диплом) есть форма заявки в формате *.pdf. Это конечно замечательно, но файл *.pdf не предназначен для изменения и правки. И что прикажете с ним делать? Распечатать и потом заполнять бумажку шариковой ручкой? Нелогично как-то, особенно учитывая, что логи ныне на компьютере и создать файл заявки (в формате *txt  или *.csv)  на диплом трудности не представляет. Трудность представляет то, что формы, генерируемые логами, как правило непригодны для отсылки в дипломный комитет (там требуют именно свои, установленные формы, а нестандартные заявки могут и назад вернуть). 
     Имеющаяся на том же сайте форма заявки в *.txt для заполнения на компьютере практически непригодна. Сомневаетесь? Попробуйте - ни форматирования, ни шрифтов. Вряд ли это вам понравится.
      Чтобы решить эту проблему сделал стандартную форму заявки на  DXCC в формате MS Word.

   Заполнить эту форму на компьютере очень просто: либо руками в Word, либо вставив туда табличку, сгенерированную вашим логом. Шариковой ручкой вам потребуется поставить лишь подпись.
   Если в заполнении формы возникнут вопросы, разрешить их поможет приведенная ниже шпаргалка. Наведите курсор на непонятный пункт и всплывёт подсказка, что это такое и как с ним бороться. Ибо родное описание от ARRL  мягко говоря, понятностью не блещет.
   В подсказке  синим дан перевод (местами вольный по форме, но точный по содержанию), зеленым - мои комментарии к данному пункту.

 Красным в тексте бланка-шпаргалки приведен образец заполнения (у вас это будет по-другому).

- Do not use this form for plaque or pin orders
- Return postage is required for the return of cards and  written requests
- DXCC accepts most credit cards. If you are not sure of the correct
charges, you may use a credit card. This will allow us to charge the
exact amount. You must clear previous balances (per your last credit
slip) with this submission in order to avoid delays.
- DXCC cannot bill you

  Note: Cards may be submitted directly to ARRL or checked by a DXCC Card Checker. If cards are sent direct to ARRL, it is not necessary to fill out this form. This form must be completed if a Card Checker checks the application.   In either case, the cards or listed credits must be sorted first by band then by mode. Cards indicating multiple contacts must be placed together. If cards with multiple credits are submitted direct to ARRL, a notation must be made on each card indicating which credits are to be entered. If no indication is made on a card, all credits will be entered into your record.
  If you fill out the form, supply all information as requested. Be sure to use the Entity name, not just the prefix.
